 • Caused by atrial contracting against a closed tricuspid valve

 • Atrioventricular dissociation

 • The cannon a wave usually occurs intermittently

 • When it is occurs with every beat, it indicates AV dissociation + retrograde atrial activation from SVT (e.g., junctional rhythm)
