HAS-BLED Score

 - Hypertension: Baseline SBP > 160mmHg

 - Renal disease: Cr > 2.6mg/dL, on HD, or s/p renal transplant

 - Liver disease: Cirrhosis, or total bilirubin > 2x ULN, or AST/ALT/alkphos > 3x ULN

 - History of stroke

 - History of major bleeding

 - Unstable INR: Time in Therapeutic Range < 60%

 - Age > 65

 - Concurrent use of antiplatelet meds or NSAIDS

 - Consumption of ≥ 8 alcoholic drinks/week
