Antiplatelet Medications - Summary Table



Aspirin - Inhibits cyclooxygenase, preventing formation of thromboxane A2

 • Acute coronary syndrome (in combination with P2Y12 inhibitor and anticoagulation) 

 • Acute stroke 

 • Primary and secondary prevention of cardiovascular disease 

 • Alternative to anticoagulation in a-fib

P2Y12 Inhibitors (clopidogrel, prasugrel, ticagrelor) - Inhibits the P2Y12 ADP receptor, preventing platelet activation

 • Acute coronary syndrome (in combination with aspirin and anticoagulation) 

 • Secondary prevention of stroke 

 • As an alternative to aspirin  in aspirin-allergic patients

GP IIb/IIIa Inhibitors (eptifibatide, tirofiban, abciximab) - Inhibits the GP IIb/IIIa receptor, preventing platelet aggregation

 • Limited use peri-PCI in acute coronary syndrome

Dipyridamole - PDE5 inhibition (and PDE3 inhibition to lesser extent)

 • Secondary stroke prevention (usually in combination with aspirin) 

 • Use in myocardial nuclear imaging

Cilostazol - PDE3 inhibition

 • Peripheral artery disease
