Veltassa (Patiromer) 

MECHANISM: Potassium Binder 

INDICATION: Treatment of Hyperkalemia 

FORMULATION: Powder for oral suspension 

DOSING: 8.4 g once daily 

Increase by 8.4 g increments per week, up to 25.2 g if needed, based on K level 

COUNSELING: Mix in 1/3 cup of water, separate from other oral drugs by at least 3 hours 



Veltassa (patiromer) is a potassium binder that can be used for patients who have chronic hyperkalemia. This condition is commonly seen in patients with CKD, HF, or diabetes. In some of these conditions, RAAS meds (i.e. ACE inhibitors, etc.) are recommended and have strong benefits, but they carry the risk of hyperkalemia. Veltassa can be used and it is taken as an oral suspension once daily (flavorless). Important counseling point to separate this from other drugs by 3 hours (before and after) to avoid interactions. Most common side effect is GI upset (constipation) and potential hypomagnesemia.
