Undifferentiated critical congenital heart disease: Patterns of Presentation 

Pink baby:

 • Presents at 1-6 mo 

 • increased pulm. blood flow 

 • HF sx (tachypnea, FTT) after PVR falls; BNP useful for dx 

 • L -> R shunt: PDA, VSD, AV canal defect, pink Tetralogy of Fallot 

Blue baby:

 • Hyperoxia test for cyanosis determines if cardiac or pulmonary source 

 • Pre/post ductal CCHD screens catch blue baby defects most often 

Gray baby: 

• Presents < 1 month 

• Decreased systemic blood flow 

• Circulatory collapse 

• Left-obstructive ductal dependent lesion: hypoplastic left heart, coarc, interrupted arch, aortic stenosis 

• Ddx: ALCAPA
