Pivot Shift test for Knee Instability

The patient lies supine while the examiner grasps the leg by the foot. While applying a valgus stress and an axial load to the foot, the examiner internally rotates the tibia while flexing then extending the knee.

The test is positive if there is subluxation of the tibia relative to the femur, indicating likely ACL Injury.
