Anti-GIomeruIar Basement Membrane Disease (Goodpasture's Disease)



What is it? A small vessel vasculitis with circulating antibodies directed against the glomerular basement membrane and alveolar basement membrane, resulting in rapidly progressive glomerulonephritis and/or alveolar hemorrhage



Diagnosis:

 • Anti-GBM antibodies in one of the following: Serum, Kidney biopsy

 • OR pathognomonic pattern on kidney biopsy

Treatment:

 • Pulse dose steroids (typically 1000mg QD for 3 days)

 • Immunosuppression: Cyclophosphamide is 1st-line, Rituximab is 2nd-line

 • +/- Plasmapheresis: Daily sessions to clear active antibodies for 2-3 weeks
