Acute Gout Flare
 1) colchicine, NSAIDs, glucocorticoids
 2) Anakinra (IL-1 inhibitor)
When to Start Urate Lowering Therapy (ULT)
 1) presence of tophi
 2) radiologic damage 2/2 gout
 3) 2+ gout flares/year
 4) consider with 1st flare if sUA >9, CKD ≥3, or urolithiasis
Urate Lowering Therapy:
 1) allopurinol
 2) febuxostat (Uloric)
 3) pegloticase
