Adrenal Insufficiency Diagnosis Algorithm
Test 1: Baseline AM Cortisol 
 - Beware of major normal value
 - variation between assays
 - Assay cross reacts with hydrocortisone
 - Falsely elevated in high globulin levels (OCPs, pregnancy)
Test 2: ACTH Stimulation
 - Cutoffs are the same as above
 - Do NOT use incremental increase to assess response
