Bradykinin-mediated Angioedema

Clinical features: Submucosal oedema (face, extremity, genitalia), Laryngeal dyspnea (25% of deaths), Recurrent attacks of abdominal pain

Types: 

 • Hereditary, 

 • Acquired: lymphoproliferative disorders (MGUS), autoimmune disease (lupus)

 • Iatrogenic: ACEI (black people, concomitant DPP4 or rapamycin inhibitors), thrombolytic therapy (ischemic stroke, female, ACEI)

Bradykinin vs. histamin-mediated AE:

 • Absence of atopic disorders

 • Genitalia involvement, abdominal pain

 • Absence of pruritus, urticaria, low BP bronchospasm

 • Duration of attacks: >24 hours

 • Steroids/antihistamines ineffectiveness

 • Specific treatment effectiveness
