Assessment:

 • Collection and interpretation of data

 • Outputs/goals: formulation of situation, factors relevant to patient goals, factors giving prognosis

Rehabilitation planning

 • Team & patient agree: formulation, overarching goals, actions

 • Outputs/goals: coordinated series of actions, involving team and others, working towards goals, long- and medium-term

Interventions of two types: 

 • Care/ support, to maintain safety and well-being, treatments, to alter outcome

 • Outputs/goals: Safety & well-being of patient & others preserved, patient's level of activities improved above expected natural history

Review

 • Reassess: progress against goals, any change in situation

 • Output/goals: May have: reiteration of cycle; planning, plan to transfer care to new setting/ team plan to discharge with self-management
